I use a UPS store location for this. My boyfriend lives in MI and I visit him every month or every couple months, so I just have stuff shipped to my UPS store and then pick it up when I'm there. Most of them will also do flat rate mail forwarding if you email the specific store and ask. Usually it's $5 to accept a package + shipping costs.

Actually, I just checked at it doesn't vary from country to country, only the shipping rates. However, your deal is actually the Premium deal - it cost $7 per month/$60 per year and not the standard which is free (supposedly). Link: http://myus2.myus.com/Memberships.aspx

Have you tried aramex shop & ship?
I have and I swear by them. They are fast and provide you with everything even a phone number.
I use it for my shipping from saks, neimanmarcus, amazon etc.

Since they use DHL/Fedex, do you get charged those fees as well? Also, can you ask about the value to be declared on your package [re: customs]?

Yes and you can estimate the cost of fedex/dhl on their website based on the weight of the package and destination. usually for a 2 pounds package it's around 40$.
You have to declare the full value (you can actually cut it down a little bit according a range they estimate)
