Yeah there's a difference.

I think movies like Saw or Hostel and other horror films are more gore than anything. They don't disturb me.

Films and books that address difficult subject matter (like rape, violence, animal cruelty, human trafficking, psychological illness, drug abuse, etc) in very realistic and graphic ways disturb me.

Films like Black Swan are good, but I didn't find it disturbing or cringe-worthy.
A film like Irreversible, which features a 10 minute long rape scene in which the camera does not cut away at any point really disturbs me.
That film also features a lot of disturbing masochistic/sodomy scenes. I will never watch that movie again.
