As Ghassan mentions on his site, this is Brooke Shields by Francesco Scavullo in 1994, wearing Kevyn Aucoin makeup - he was the first artist to tweeze her eyebrows (theghay.tumblr.com "The Ghay"

The thing that impressed me about Scavullo was his kindess to Gia. It was to be her last shoot for the cover of Cosmo. He carefully placed her arms behind her back so no one could see the needle tracks. He did it in such a way that made her look wonderful and praising her all the while.
