The first one is Carolyn according to the website, though it does look very much like Karen.
I wish they'd use newer models...

at least its better than those lame pregnant covers where the celebrity mother to be is holding their stomach surrounded by flowers and their family

It's British.I think it started in the 90s/end of the 80s, not sure though.Funny you haven't heard about it, it's really famous.

Is there anything about the models inside? eds? Isn't that pic of Liya quite old, or is she pregnant again?
