Q1: So far NO

Q2: I think we'll be expecting another one, the pic she twitt with Sasha was on october 5th I think is too soon for the november issue. And the fact they were together the same day probably is because they'll have pictures together.

Need to mention that inside Vogue Spain, there's two un-posted shots by Inez & Vinoodh. Both are profile photos, rather than full-frontal which the editorial consists of. I'm too scared to scan, incase I mange to crease the magazine or whatever. So these less than satisfactory iPhone photos will have do do:
