A horrifying amount of them actually are. I've seen so many small to mid-sized brands show a custom one-off design, expliciting state that it's custom and still have people ask "wHeRe CaN i FiNd ThIs DrEsS?".I've read of stylists, even big names, having little to no control over adjusting looks. Of course they will take a celeb's sensibilities into account but I think a lot of brands are afraid, or think consumers are too stupid, to have a celeb alter or adjust a garment and then a consumer who likes it can't find it in stores exactly as it looked on the red carpet. Like they really treat celebs as walking mannequins and expect them to be grateful for the opportunity.
