The face is too retouched, the contours of the face have gone making her too flat, (especially the neck), which you've had to bring back by shading the side of the face. In the original she was really pretty, but it looks like you've had some one else in mind when reshaping her face. A lot better than many I've seen though still too much!

it's very easy though .. what part are you stumped on...
you make a new file, choose the size of your canvas (on User CP it writes the max. size of your avatar is 160 pixels by 80 pixels)

and then draw on your canvas or paste pictures inside , etcetera

When I make an avatar I just crop the image to the size i want (for example 100x100px).. it's super easy just open the image and crop. Voilà
