The problem is simple: he is a bipolar; and he refuses to take his meds...so his brain does not work as it should. The moment he comes back to meds, he will come back to normal (well, sort of)...

I'm sure bipolar disorder manifests differently for different people, but generally speaking I think you see people going through highs and lows, manic episodes and depressive episodes. And at least from personal experience of having people close to me with BPD, there's often a sense of shame or embarrassment after the mania and if they're self-aware, even a sense of fear as they feel it coming on. I don't feel like we see that spectrum of behavior from him. He's just self absorbed "Ye" all the time and then when he doesn't get his way or feels wronged, he lashes out. Obviously we don't have access to his behavior around the clock, and I don't feel comfortable playing doctor and trying to diagnose exactly what's going on, but I think he's kind of just a narcissistic d!ck. With an ego so massive and having been for so many years surrounded by enablers and yes men, he cannot handle criticism. And it's only gotten worse as he sees how often he not only gets away with awful behavior but is actually rewarded, financially and with attention. And he will only keep escalating things to get that attention. As people grow used to his antics, he'll have to up the ante. I don't think he can hide behind BPD and be given a pass every time. Many people deal with similar issues and still manage to take accountability and do what needs to be done in terms of treatment and medication to mitigate damage. He takes zero accountability.

Yes, I agree with you it could very well be a combination of two factors for his behaviour: a mental illness (be it BPD or any other); and a narcissistic, megalomaniac personality.
