The best model + model collaboration is Gemma Ward and Lily Donaldson, in my opinion. They compliment each other so well and both are highly effective in print (and I don't even like Lily!)

The best model + photographer collaboration will forever, in my mind, be Guinevere van Seenus and Paolo Roversi. Both are just so perfect for one another - Paolo's dreamy photography and Guinevere's near perfect and lush features just fit hand in hand so well. Love it.
