the cover of Vogue Portugal is the reprint of Vogue Nippon but still I'm looking for Portugal cover

and as for cover model I'm pretty sure it's Caroline!!

It is actually Lily Donaldson. The cover is a reprint of "The Wonder Shape" by Greg Kadel for Vogue China March 2006.

According to fmd1.com, Doutzen Kroes was on the cover of Vogue Portugal July 2007, and Karolina Kurkova for June 2007. Can't find any photos, though.
