I have a baby this happens all the time!! It's awkward when your not wearing your best bra

When my boy was wee, I have to say that I didn't have this problem of my bra hanging out all the time.

However, I think it's the way that tank tops are cut of late; they have such deep armholes and necks, and suddenly it's fashionable to have bra-straps and the like showing. So I think Sienna is a bit of a victim of the style right here.
