I found some information on the inspiration for the the whole visual of the Joker. The costume designer said she drew inspiration from Johnny Rotten
and the costume designer said Chris Nolan brought in a book of Francis Bacon
while they were working on the make up. Some pictures:

This is said to be the early conceptual art for the Joker released on the Art of The Dark Knight book...The early concept is even cooler than the result... Ok I'm a little fixated on The Joker right now...
